HUGH BOYD REFERENCE LETTER FORM

Students who wish to have their teachers write a reference letter or be nominated for a scholarship MUST fill out this form prior to talking to their teacher requesting a reference. The form should be filled out in its entirety in order to provide your reference with enough information to write your letter. Requests for a reference letter must be given to the teacher a minimum of two weeks before the student needs it or the student risks the teacher being unable to do the reference.
